General Description

This position is located at the LSWN Clinic, Little Axe, OK.  The LSWN Clinic is an outpatient multi-disciplinary clinic established and managed by the Absentee Shawnee Tribe of Oklahoma. The LSWN clinic is an ambulatory clinic that provides direct care services to Native American patients.  The Administrative Assistant/LPN provides administrative support for field staff and Home Health Administrator/ Director of Nursing.  Perform duties as LPN when required.

Responsibilities

  • Enters patient referrals within the required time frame.
  • Maintains survey compliance book.
  • Answers phone calls and routes to the appropriate staff member or takes message and ensures that the message is received.
  • Monitors, tracks and orders office and medical supplies.
  • Ensures that new hire paperwork is completed copied and sent to the corporate office.
  • Maintains personnel files and keeps filing up to date.
  • Enters attendance/absentee information into Home Care Home Base.
  • Processes discharged patient surveys on a weekly basis and summarize patient survey results on a monthly basis.
  • Maintains updated census reports.
  • Tracks telehealth monitoring equipment and cleans units as they come in practicing infection control and universal precautions to ensure the safety of the patient.
  • Documents case conference minutes and updates case conference tracking booklet.
  • Tracks and inspects company vehicles and reports any issues or violations of the company Car Policy.
  • Schedules visits to appropriate staff
  • Verify all paper visits and attach to patient chart.
  • Manage weekly scheduled for all staff.
  • Monitor therapy reassessment visits.
  • Write therapy orders for scheduling in HCHB.
  • Lives the PLUS Standards in the care of patients, daily tasks and interactions with others.
  • Maintains regular attendance and punctuality which are critical in order to complete the day to day tasks of this position.
  • Provides nursing care and teaching to patients and families when required.
  • Collects and distributes office mail.
  • Other duties as assigned.

Education Requirements and Qualifications

Native American Preference/EEO/Drug Free Workplace.
EDUCATION AND EXPERIENCE:
Requires a high school diploma or GED.  Must have basic knowledge of Word, Excel, e-mail programs and other general office equipment.  Must have minimum of 2 years of experience as an LPN withhout discipline or restrictions.  Must be able to communicate information regarding a patient’s condition to the appropriate staff members.  Must be able to deal tactfully and cheerfully with patients, family members, visitors and staff.  Must be able to function independently and make independent decisions when appropriate.
SKILLS AND ABILITIES:
Knowledge of fundamental nursing care principles, practices and procedures. Must have a basic knowledge of adverse symptoms and possess the ability to react in emergencies. Must have a general knowledge of the human anatomy and adverse signs and symptoms. Ability to relate well with patients and their families and the ability to teach.
ESSENTIAL PHYSICAL REQUIREMENTS:
Continuous sitting, occasional bending/stooping, walking, kneeling, and standing.  Infrequent balancing and pushing/pulling.
WORKING CONDITIONS:
Constant contact with families, patients, visitors and working in areas throughout the office.  This position is subject to frequent interruptions, pressure from multiple/emergency calls and hostile or emotionally upset patient, family members, personnel and visitors.  This position may work some evenings/weekends/holidays or other shifts as necessary.
Must be able to pass a background check and drug screen.
