General Description

The director of nursing is responsible for the overall coordination of home health services provided to clients with an emphasis on quality care, positive outcomes, continuity of services and financial accountability.  This position is responsible for agency oversight and acts as the central contact and director of the branch.

Essential Duties & Responsibilities

  • Manages and supervises the day-to-day office and clinical operations.
  • Ensures that patient needs are met and acts as a resource of field staff.
  • Monitors and completes all task screens.
  • Reviews and follows up on complaint forms and all QI reports.
  • Conducts evaluations and develops performance improvement plans as needed.
  • Holds weekly Case Conferences to discuss client issues.
  • Works with Human Resources to recruit, hire and train qualified employees.
  • Ensures that newly hired nurses are given a complete orientation.
  • Ensures that all employees practice infection control and universal precautions.
  • Educates employees on recognizing and reporting abuse and taking appropriate actions according to policy and federal/state laws.
  • Manages branch’s annual budget and is accountable for revenue/expenses each month.
  • Takes responsibility for financial impacts of all admissions, discharges, and recertification’s analyzing the impact of each one as they occur.
  • Monitors staff productivity.
  • Lives the PLUS Standards in the care of patients, daily tasks and interactions with others.
  • Maintain regular attendance and punctuality which are critical in order to complete the day to day tasks of this position.
  • Completes initial and ongoing comprehensive assessments of the patient’s needs, including OASIS assessments at the appropriate points in time.
  • Initiates the plan of care and revises as necessary under the direction of the physician.
  • Provides those services and/or treatments requiring substantial and specialized nursing skills.
  • Assesses and educates the patient and family/caregiver regarding the disease process, self-care techniques, and prevention strategies and nursing related needs.
  • Initiates appropriate preventative and rehabilitative nursing procedures as ordered.
  • Prepares clinical and progress notes.
  • Coordinates services and refers to other services as needed.
  • Informs the physician and other staff of changes in the patient’s condition and needs.
  • Evaluates the effectiveness and outcome of care.
  • Supervises, delegates, and teaches duties and tasks to Licensed Practical/Vocational Nurses, Home Health aides and others providing services, as appropriate.
  • Plans for discharge from service.
  • Participates in in-service programs.
  • Re-evaluates the patient’s nursing needs on a regular basis.
  • Accepts and carries out physician orders both oral and written.
  • Assists physician in evaluating the patient’s level of function.
  • Practices infection control and universal precautions to ensure the safety of the patient.
  • Other duties as assigned by the Home Health Administrator.

Education Requirements and qualifications

  • Must have a RN license in good standing from the State of Oklahoma.
  • Minimum of two years nursing experience and one year of home health experience.
  • Home Health, Critical Care or Emergency Room experience preferred.
  • Requires CPR certification.
  • Must possess knowledge of the Medicare guidelines governing home health agencies.
  • Must have a valid Oklahoma driver’s license, reliable transportation and vehicle insurance coverage.
  • Must be able to travel long distances and handle the stress of frequent scheduling changes.
  • Must have basic knowledge of personal computers.
Physical Requirements
  • Must be able to lift, push, pull and carry up to fifty (50) pounds.
  • This position will require lifting, pushing, pulling and carrying more than 20 pounds up to 70% of the time.
  • This position will require lifting, pushing, pulling and carrying more than 30 pounds up to 25% of the time.
Working Conditions
  • This position will have constant contact with employees, patients, families and visitors.  The DON/RN Case Manager will work in patient’s homes, or other healthcare facilities and offices as needed.  This positon will work alone in the patient’s home and may need to assist patients with ambulation. This positon will travel in all weather conditions, there is potential for exposures to infectious diseases including AIDS and Hepatitis B viruses.
