GENERAL DESCRIPTION
This position is located at the Behavioral Health Services Department, Little Axe Health Center. The incumbent will provide direct outpatient clinical, prevention, and educational services to children, adolescents, and adults; to include crisis intervention to the members of the “Following in our Footsteps” MSPI Program, their immediate dependent family members, and members of other federally-recognized American Indian Tribes and Alaska Natives.

RESPONSIBILITIES
 Provides comprehensive, clinical assessments. Assessments will be comprehensive considering the overall clinical disposition of the patient, as well as strengths, needs and preferences. A clinical diagnosis will be given with a detailed description of the current symptoms and needs. Treatment recommendations/plan will be made based on a thorough assessment. Outpatient Therapists may only diagnoses and provide services within their scope of training and expertise.
 Meets with patients to obtain information pertinent to providing outpatient mental health counseling services.
 Designs the formulation of each client’s over all treatment plan, discharge criteria, and continued service recommendations via collaboration with other professional providers with appropriate consent regarding patient care.
 Utilizes a combination of individual and group counseling sessions to help the patients, and to observe their attitudes and readiness for accepting guidance.
 Meets with the MSPI family members to educate them on the most useful role that they can play in the lives of their families and youth. Make presentation on the dangers of Meth and Suicide.
 Assist with other MSPI staff members in the prevention/intervention activities for individuals, families, and the community.
 Maintains encounters within EHR, documents any and all clinical services, communication, and other client related contacts in the electronic health record within 24 hours of service delivery. Documentation shall be in accordance with the Medicare/Medicaid requirements as well as with the current policies and procedures of the health system.
 Maintains Federal, State, and Local guidelines related to duty to report, and protection of Confidentiality, Ethics, and Protected Health Information Act (HIPPA) to the patients.
 Fulfill any and all requirements established by the relevant Board in the State of Oklahoma to maintain required clinical licensure/certification. Maintain necessary continuing education requirements in accordance with Core Services requirements and/or relevant to Oklahoma State Licensure/Certification.
 Documents daily work load, contacts, etc. and will submit a monthly report.
 Performs other related duties associated with the MSPI Program activities and assist the Project Coordinator with those activities.

EDUCATION/QUALIFICATION REQUIREMENTS
 Must have Master’s degree in Mental Health or related field from an Accredited University; and have License for an LCSW, LMFT, LPC; required.
 At least one year of working experience in the social services in the mental health field.
 Have the ability to comprehend and produce accurate clinical documentation, including, but not limited to, patient treatment plans, service notes, and progress summaries.
 Have the ability to communicate professionally, (both verbally and in writing) with relevant individuals, including, but not limited to, patients, parents/guardians, colleagues, and other mental health providers.
 Knowledge of Federal, State and Tribal Social/Mental Health Programs; Knowledge of tribal culture and traditions.
 Must possess a valid Oklahoma Drivers License.
 Must be able to pass a criminal background check.
