General Description
This position is located within the Absentee Shawnee Tribe Health Programs at the Little Axe location. The home health aide provides services that are ordered by the physician in the plan of care and directed by the RN Case Manager. The Home Health Aide will provide hands-on personal care, perform simple procedures as an extension of therapy or nursing services, assist in ambulation or exercises, and assist in administering medications that are ordinarily self-administered. Travel is required 100% of the time.
Responsibilities
Primary Duties include, but not limited to:
Assists patients with bathing, oral care, hair care, shaving, nail care and dressing.
Helps patients with cooking and feeding needs.
Takes vital signs as assigned.
Notifies the nurse of any changes in the patient’s conditions or needs.
Assists patients with ambulation.
Opens and closes medication containers for patients.
Helps with transferring patient and rehab services after therapy and provides relevant instructions.
Works with team members to ensure HHA visit does not occur at the same time as another discipline.
Practices infection control and universal precautions to ensure the safety of the patient.
Recognizes and reports any form of abuse taking appropriate action according to policy and federal/state laws.
Lives the PLUS Standards in the care of patients, daily tasks, and interactions with others.
Maintains regular attendance and punctuality which are critical in order to complete the day to day tasks of this position.
Other duties as assigned
Marginal Job Functions:
Works in the office as needed or sets up telemonitoring equipment when patient census is low.
Participates in training activities.
Assists with orientation of new Home Health Aides as requested.
Education Requirements and Qualifications
Education and Experience Requirements:  Active HHA Certification in good standing with the Oklahoma State Department of Health.  Requires CPR Certification within 60 days from date of hire.  Reliable transportation, proof of insurance and an active Oklahoma Driver’s License in good standing.
Essential Physical Requirements:  The Home Health Aide works alone in the patient’s home and may need to assist patients with ambulation.  The Home Health Aide must be able to lift, push, pull and carry up to fifty (50) pounds.  This position will require lifting, pushing, pulling and carrying more than 20 pounds up to 70% of the time.  This position requires lifting, pushing, pulling and carrying more than 30 pounds up to 25% of the time.
Working Conditions:  Constant contact with families, visitors, patients and employees.  Will work in patient homes, other healthcare facilities, and offices.  This position travels in all weather conditions.  There is potential exposure to infectious diseases including AIDS and Hepatitis B viruses.  (See Physical Demands for more information).
