General Description
This position is located with the Absentee Shawnee Tribes Health System. The purpose of this position is to manage the Special Diabetes Program for Indians (SDPI) Grant with the Indian Health Services and coordinate the activities of the Diabetes and Wellness Program. This also includes providing patient care specifically to the Absentee Shawnee Tribe’s diabetic population and promoting comprehensive diabetes care for patients. This position is responsible for promoting SDPI Best Practices and a systematic approach to diabetes care.
Responsibilities & Duties
Supervision of SDPI staff
Reporting for the SDPI grant
Oversight and management of goals, objectives and outcomes of SDPI Best Practices
Conduct assessments & evaluations of diabetic patients.
Educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing.
Collects patient data to monitor health status and proactively manage the patient's condition.
Ensures data is gathered and accurately documented demonstrating the utilization of the appropriate process to promote patient outcomes.
Work closely with professional providers and other clinic staff in planning and evaluating patient needs and care.
Participates in activities promote professional growth and self-development. Attends pertinent in-services, departmental and other meetings as requested by the immediate supervisor.
Conduct follow-up with patients.
Assist patients in developing and meeting goals and areas of need and assist in developing treatment plans and assesses them regularly.
Assess patients’ formal and informal support systems; evaluate effectiveness of services provided, treatment and medication.
Independently manages activities of caseload.
Performs other related duties as assigned.
Education Requirements and Qualifications
Bachelor’s degree or equivalent in Health Sciences or related field.
Experience in lieu of Education may be considered.
Dietetic Registration from the Commission on Dietetic Registration.
Licensure of Dietetics from the Oklahoma Medical Board of Licensure and Supervision.
Must have valid registration as a Dietitian Nutrition with the Commission on Dietetic Registration
Must have a valid license as a Licensed Dietitian with the Oklahoma Medical Board of Licensure & Supervision
Minimum of two (2) years of experience working with diabetes patients, diabetes education and case management required. Experience in Native American health care programs is preferred.
Demonstrated ability to communicate effectively with peers and superiors.
Demonstrated knowledge of Federal regulations, guidelines, and legislation related to diabetes care programs.
Must be able to speak in front of groups and to communicate in writing policies, procedures, memoranda and training materials.
Must have excellent interpersonal skills and be culturally sensitive to Native population and needs.
Ability to operate and communicate effectively while under pressure is essential.
Must be able to professionally relate and communicate effectively and efficiently with the public and professional providers.
Ability to manage multiple concurrent projects.
Ability to recommend and initiate program improvements and modifications to achieve mission goals.
Must have excellent computer skills and familiarity will Microsoft Office Suite.
Must be able to read, understand, apply, and retain knowledge of departmental rules, regulations, and policies.
Must be able to follow all instructions, written and oral.
Must have a valid Oklahoma Driver’s License.
Must be able to pass a background check.
Must be able to pass a drug test.

Must be able to sit, stand, stoop, bend or kneel for long periods of time.
