General Description
The purpose of this position is to serve all of the AST Health System.  The AST Health System is an ambulatory health system that currently serves patients with Insurance (Indian and Non-Indian), Absentee Shawnee Tribal Citizens, and Absentee Shawnee Tribal employees and other Native Americans.  
This position provides frontline services (home visits) to patients living with conditions of Alzheimer’s and other dementia related diseases.  The Home and Community Based Services’ Dementia Care Specialist (HCBS DCS) provides a variety of general and specialized support care for patients who are or require home-based care.  The HCBS DCS will deliver culturally appropriate information; provide dementia-specific consultation; and technical assistance related to cognitive screening, individual and family caregiver issues, and assists with various related resources.  Additionally, the HCBS DCS will facilitate dementia friendly community efforts deliver dementia and program services training, and support people with dementia and their family members.  Collaborate with and work in concert with providers, nurses, and healthcare professionals.  Regular travel is required to meet the needs of the position.
Responsibilities
The HBCS DCS will work with families, caregivers and individuals with Alzheimer’s disease and related dementias. HBCS staff will carry out this work through the outreach and delivery of evidence-based services that provide person-centered dementia care to community residing tribal members. The use of designated intervention tools to support and aid caregivers in the home-based setting.
Facilitates effective training programs for a variety of participants.
Coordinates the development and/or distribution of all dementia related training materials, and maintains the training calendar.
Must be able to successfully teach and deliver dementia training programs in-person, group sessions and virtually.
Conduct patient assessments, screenings and evaluations.
Educate patients on the roles of healthcare teams; available assistance; and levels of health care. Assists with the navigating through the processes caregivers, patients and families encounter when providing home-based care.
Outstanding customer service skills and desire to positively impact caregivers’ and patients’ lives by providing caregivers’ tools that support and help the caregiver meet day-to-day activities and challenges.
Promote communication and collaboration among the dementia health care teams.
As a part of the Caregiver Support, the HBCS DCS stays informed regarding current federal and state legislation directed at assisting caregivers to continue to provide caregiving in an environment that is safe for both themselves and the patient.
Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services and ability to coordinate services.
Must deliver assessments to caregivers and patients and maintain and deliver program reports to management.
Make knowledgeable recommendations to maintain and improve the program.
Confers with management and line of supervision to determine need and scope of training programs and results.
Uses project/time management skills to plan, prioritize and executive multiple activities.
Sound decision making and organizational skills.
Must be flexible in work schedule as work hours may vary.
Other duties as assigned.
Education Requirements and Qualifications
Associate’s degree (Bachelor’s preferred) in psychology, sociology, cultural studies, nursing, communications or other social science based field of study.
Knowledge, Skills and Abilities
Must be sensitive to Native American cultures.
Must be reliable, detailed oriented, capable of multi-tasking with excellent written and verbal communication skills as well as strong professional customer- service interpersonal skills.  
Ability to communicate effectively orally and in writing; ability to relate well with co-workers and the general public.
Ability to adjust to changing conditions.
Must be computer literate required (Windows operating systems – Microsoft Word, Excel). 
Must have a valid Driver’s License
Must be able to pass a background check and drug screen.
Must be certified in American Heart Association Basic Life Support (BLS) CPR or obtain within 6 months of hire.
Physical Requirements and Working Conditions
Must be able to sit, stand, stoop, bend or kneel for long periods of time.
Sitting or standing or walking for long period of time; occasional bending, squatting, kneeling, stooping; good finger dexterity and feeling; frequent repetitive motions; talking hearing and visual acuity.
