Community Health Aide/Caregiver Grant Coordinator

General Description
The purpose of this position is to serve all of the AST Health System. Community health workers promote wellness by helping people adopt healthy behaviors. They implement programs and advocate for people who may have limited access to health resources and social services.
The grant coordinator will be responsible for arranging services, recruiting, and providing assistance to caregivers and their families enrolled in the Caregiver Program. The coordinator will also be responsible for maintaining and providing confidentiality to participants and their records.
 
  • Responsibilities & Duties
  • Assist clients in their homes, community, or clinic setting. 
  • Document all client encounters and contracts made on behalf of clients; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client.
  • Document activities, service plans, and outcomes achieved by client in an effective manner.
  • Educate client on the proper use of the Emergency Room, and provides information for alternatives.
  • Coach patients in effective management of their chronic health conditions and self-care.
  • Assist patient in understanding care plans and instructions.
  • Motivate patients/clients to be active and engaged participants in their health and overall wellbeing.
  • Assist clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and /or social services.
  • Assist with Elder feeding program, acting as liaison between clinical and community needs and services, as necessary.
  • Provide support and advocacy during initial medical visit or when necessary to assure clients' medical needs and referrals required are being conveyed.
  • Follow up with both clients and providers regarding health/social services plans.
  • Facilitate communication and coordinate services between providers and the clients/patients.
  • Coordinate and monitor services, including comprehensive tracking of clients' compliance in relation to care plan objectives.
  • Travel extensively to client homes, community locations, various agencies, and other outreach destinations.
  • Maintain HIPPA compliance at all times.
  • Perform other duties as assigned.
  • Monitor grant program to endure compliance with contract provisions and funding source regulations.
  • Meet routinely with the Project Director and provide on-going updates.
  • Plan, coordinate, and monitor grant funded program activities and special projects.
  • Act as a liaison and resource to the tribal community, organizations and partnerships.
  • Coordinate the development, production, ordering and dissemination of educational materials and promotional items.
  • Assist in preparation of grant reports as required by grantee.
  • Perform other related duties as assigned.
Education Requirements and Qualifications
  • High School diploma or GED.
  • 1 year experience directly related to the duties and responsibilities of clinical health and human services.
  • Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
  • Must have American Heart Association Healthcare Provider CPR and First Aid or able to obtain within 90 days after hire.
  • Knowledge of community agencies and resources.
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.
  • Demonstrated proficiency in grants management, public speaking, group facilitation, training, prevention program development, prevention evaluation, strategic planning, and implementation, and data compilation.
  • Understand principles, practices and techniques of grant project planning, development, evaluation, procedures of budget preparation and monitoring, and contracted services.
Knowledge, Skills and Abilities
  • Must be reliable, detailed oriented, capable of multi-tasking with excellent written and verbal communication skills as well as strong professional customer- service interpersonal skills.  
  • Ability to adjust to changing conditions.
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Ability to plan, implements, and evaluate individual client care plans.
  • Ability to communicate medical information to health care professionals and care coordinators over the telephone.
  • Must be computer literate required (Windows operating systems – Microsoft Word, Excel). 
  • Must have a valid Oklahoma Driver’s License.
  • Must be able to pass a background check and drug test.
Physical Requirements and Working Conditions
  • Must be able to sit, stand, stoop, bend or kneel for long periods of time.
  • Prolonged sitting, standing, or walking; occasional bending, squatting, kneeling, and stooping; good finger dexterity and sensory perception; frequent repetitive motions; as well as talking, hearing, and visual acuity are required.
