General Description
This position is located at the Little Axe Health Clinic, Norman, OK.  The Little Axe Clinic is an outpatient multi-disciplinary clinic established and managed by the Absentee Shawnee Tribe of Oklahoma. The Little Axe clinic is an ambulatory clinic that provides direct care services to Native American patients. The purpose of the position is to provide professional nursing care to a patient population exhibiting a variety of illness and injury.
Provide employee health screenings and maintain all clinic employee health records, including documentation of immunizations in OSIIS and other required recordkeeping systems.
May provide training for Basic Life Support CPR training to all clinic employees and maintain records.
Provide health promotion education as indicated and/or ordered by primary care provider.
Assist in coordinating and providing community outreach influenza and other immunization clinics as needed.
Provides screening of patients to obtain preliminary health histories, collect data on patients’ condition using proper nursing protocols, recognizes and reports any unusual or abnormal behavior that may indicate a need for urgent care.
Documents within the patient’s medical record any nursing observations and data gathered, nursing interventions, therapeutic measure, and nursing notes related to the medical care of the patient.
Assists in community outreach by completing home visits, evaluating home safety, medication planners and other health needs of the patients.  Delivery of assistive devices.
Assist in completing nursing home visits of our population.  To complete hospital visits on patients to collect information on any discharge needs and to facilitate in meeting discharge needs. 
Assist in executing grant and other program initiatives as assigned to the department.
Provides assistance to the Clinic Medical Director and other primary care providers with medical care and treatment of patient to ensure quality and continuity of care within the parameters of appropriate professional nursing standards.
Promotes continuity of care by providing medically relevant health care instruction and patient education.
Upon designation, functions as assistant to the Clinic Infection Control Nurse and helps to provide training to the Clinic staff for infection and hazardous exposure control including exposure determinations, eliminating and reducing exposure risks, hazard communication, handling of infectious waste, post-exposure procedures and record keeping.
Participates in continuous quality improvement activities including quality assessments, surveys, and checklist assessments. Also participants as a member of the Clinic Safety and Security Committee.
Provides written reports to the Public Health Director, Clinical Services Director, Medical Director, and/or Executive Director, Health as may be required.
Provides assurance of Confidentiality of Medical Records.
Upon designation, functions as the Safety Trainer for the Clinic Safety and Security Committee and performs training sessions to assure compliance with OSHA regulations.
Performs other duties as may be assigned by the Public Health Director, Clinical Services Director, Medical Director, and the Executive Director, health.
Education Requirements and Qualifications
Graduate of an accredited school of nursing with a degree and certified as a Licensed Practical Nurse.
Knowledge, Skills, and Abilities
Must have at least two (2) years of professional nursing experience, experience covering a wide range of nursing services from screening, to patient education, counseling, and treatment of minor and chronic illness.
Must have a professional knowledge of nursing care principles, practices and procedures, working knowledge and application of proper nursing protocols, training and knowledge of pharmaceuticals and their side effects and complications of use.
Ability to observe and react to adverse signs exhibited by patient and the ability to respond appropriately in emergency situations.
Able to educate and counsel with patient regarding their medical care and treatment as ordered by a physician.
Must be certified American Heart Association Healthcare Provider CPR or be certified within 6 months of hire..
Must be able to effectively relate within local community.
Must be able to respond effectively to oral and written directives.
Must have a valid Oklahoma Driver’s License.
Must be able to pass a criminal background check.
Physical Requirements and Working Conditions
Must be able to sit, stand, stoop, bend or kneel for long periods of time.
Sitting or standing or walking for long period of time; occasional bending, squatting, kneeling, stooping; good finger dexterity and feeling; frequent repetitive motions; talking hearing and visual acuity.
