General Description
The Transitional Housing Coordinator provides supportive services to participants and their children with their overall goal of increasing independence, housing stability, and safety. This position coordinates the daily operations of the transitional housing program. Assists victims who are transitioning from incidents of domestic violence, sexual assault, dating violence, sex trafficking and/or stalking.
Responsibilities
Responsible for day-to-day operations of the Transitional Housing Program and reports directly to the Director regarding the program’s progress.
Maintains confidential records and statistical information for reporting on each victim/survivor served.
With the advice and approval of the Director, formulates and implements procedures and recommends policies designed to enhance the effectiveness of the program and to further the goals, policies and initiatives of the program.
Works with transitional housing participants on a monthly basis for case management and goal setting/status.
Review applications from and conduct intakes with program applicants.
Determine program eligibility and communicate eligibility decisions with applicants.
Design a goal plan for each TH program participant.
Provide ongoing support and communication with TH program participants.
Work closely with Accounting Specialist and Executive Director to maintain working knowledge of program budget.
Coordinate collection of TH program data and grant reporting information.
Complete and submit quarterly grant reports.
Provide safety-planning services and conduct program participant safety checks, as needed. Contacts and advocates participants regarding their particular situation.
Maintains regular office hours for meeting with participants to make it easier for the client to receive assistance. May be required to make special arrangements after hours to meet with participants for home visits or other supportive services.
Provides referrals to participants for counseling, support groups and financial assistance to better support the client.
Contacts and counsels clients regarding their particular situation.
Be thoroughly familiar with the policies and procedures guiding the work of this position in particular and the program or department overall. Perform job functions in line with these policies and procedures.
Pay bills for tribally-owned TH units on a regular/on-time basis and monitor security system for such units.
Be on-call for calls that come into the program regarding current participants of the TH program or other individuals seeking information pertaining to domestic violence, sexual assault, dating violence, sex trafficking, or stalking
Purchase and deliver emergency items to TH participants that include household items, furniture, food, clothing, etc.
In/Out State Training as permitted by funding
Other duties as assigned
Education Requirements
Bachelor Degree in Social Work or a related field preferred; or equivalent combination of education and experience.
Must possess a valid Oklahoma Driver’s license.
Must be able to pass a background check.
